Multimedia Permission Release Form

I give my consent for the Telluride Choral Society to photograph or video my child or use photograph(s) or videos that already exist of my child that were taken in a music setting. I understand that the photographs, digital images, or video segments may be used in print or electronic media and that the photographs may be displayed on websites or social media pages owned or sponsored by the Telluride Choral Society. I give the Telluride Choral Society permission to publish, exhibit, and distribute these materials. Possible uses include concert marketing, recruitment, and other actions that align with the Telluride Choral Society’s mission to educate, unite, and uplift through music. I understand that the Telluride Choral Society owns the copyright to the multimedia material in which my child may appear. The Telluride Choral Society will assure that it conveys positive images of children and reflects music education. Please complete a separate form for each participant.
